N.C.L.A. Release, Waiver and Permission to Treat form
Amature Athletic Minor Waiver and Release of Liability
In consideration of being allowed to participate in any way in athletics/sports programs and related events and activities of the New Canaan Lacrosse Association, NCLA, and as Parent(s) or legal guardians agree:
That the parent(s) or legal guardians will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Give my consent to New Canaan Lacrosse Association and the host organization of my NCLA sanctioned event to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of the minors participation in NCLA and sanctioned events.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the risks not known to us or not reasonably foreseeable at this time.
Assume all of the forgoing risk and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue the New Canaan Lacrosse Association, its affiliated clubs, their respective administrators, directors, agents, volunteers, coaches and other employees and associates, other participants, sponsoring agencies including the Town of New Canaan and the New Canaan Board of Education, sponsors, advertisers and if applicable owners and leasers or premises used to conduct the event, all of which are hereinafter referred to as “releasees†from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demand losses or damages on account of injury, including death, damage to property caused or alleged to be caused in whole or in part by negligence or the releasees or otherwise.
I/WE HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT WE GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
